I do not find the premise plausible. Years ago I worked with a tard. He had fetal alcohol syndrome, and one of the reasons everyone around him knew it was that he would not stop talking about it, and every other thing that occurred to him. (it also happened that he looked like Beetlejuice, but that isn't relevant right now) He was one of those people with no internal monologue. He seemed to be able to think only by speaking his thoughts aloud. It made it obvious that there was something very wrong with him. It also made him extremely obnoxious, though he did not know it, and he was not trying to drive everyone around him straight up the nearest wall with his constant yap-yap-yap-yap-yapping. He genuinely did not know any better and I judged him incapable of learning. He had an IQ of about 70, which became really obvious just from observing his behavior and listening to his thought process for a few minutes.
And he understood conditional hypotheticals. Now, vocabulary was not one of his strengths. You would have had to explain to him, possibly more than once, that you were asking him to tell you what would happen if something had been different. He grasped the concept.
I suppose it is possible that there exist featherless bipeds out there who walk and talk and feed and dress themselves who genuinely lack the cognitive wherewithal for this level of abstract thought. I hope I never meet one.
